Report the death of someone with a learning disability or an autistic person



Anyone can use this online form to report the death of someone who:

The information you provide when you report a death will be used by the Learning from Lives and deaths, people with a learning disability and autistic people (LeDeR) programme to better understand the needs of people with a learning disability or autistic people and to improve future services.

The form should take around 15 minutes to complete.

Before you start

The form will ask you to provide details about:

  • The person who has died
  • The circumstances of the death
  • Your relationship to the person who has died
  • Your contact details

Please give as much information as you can in the notification form, this will help us to identify the person who has died.

The national data opt-out is a service that allows patients to opt out of their confidential patient information being used for research and planning.

LeDeR has been granted an exemption from the National Data Opt-Out by the Health Research Authority Confidentiality Advisory Group to the Secretary of State. This means that even if a person had opted out via the National Data Opt-Out before their death, their health and social care data can still be used for the purpose of a LeDeR review.
